Higenamine and Mavacamten

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Mavacamten

Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with Camzyos (mavacamten). This is a medicine for hypertrophic cardiomyopathy, and your body clears it largely through a liver enzyme called CYP3A4. In a lab dish, higenamine slowed that enzyme down by about 21%. If that held true in people, mavacamten could clear a bit more slowly and build up, which matters because this is a drug with a narrow safety window. Too much can weaken the heart's pumping (reduced ejection fraction) and lead to symptoms like shortness of breath, swelling, or feeling faint.

I want to be honest though: this is theoretical and only seen in test-tube research, never confirmed in humans. On top of that, higenamine is a stimulant that can affect heart rate and blood pressure on its own, which isn't ideal with a heart medicine. Please check with your cardiologist or pharmacist before mixing the two.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Mavacamten page

“Grapefruit is the first thing I would set aside for anyone taking mavacamten (Camzyos), the medicine for obstructive hypertrophic cardiomyopathy. It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”

About Higenamine

What is Higenamine?

  • Weight loss and fat burning
  • Pre-workout energy and performance
  • Appetite suppression
  • Athletic stimulant
  • Traditional medicine remedies

Higenamine is a plant-based stimulant compound added to many weight-loss and pre-workout supplements, but there is very little human evidence that it works and real concerns about heart and blood pressure effects. It is banned in sports by the World Anti-Doping Agency (WADA), and product labels often list inaccurate amounts, so most people should approach it with caution and talk to a pharmacist or doctor first.
