Netupitant, Palonosetron (Oral) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects drugs metabolized by CYP3A4.
In vitro research shows that higenamine inhibits CYP3A4 enzymes by 21%. However, this effect has not been reported in humans.

Here's the gist for Akynzeo (netupitant and palonosetron). One of its ingredients, netupitant, is cleared by a liver enzyme called CYP3A4. In a lab dish, higenamine slowed that enzyme down a little (about 21%). If that happened in your body, netupitant could build up a bit higher than usual, which might mean stronger or longer-lasting effects like headache, constipation, fatigue, or weakness.

But honestly, this is very theoretical. It has only been seen in test-tube studies, never in real people, so I wouldn't lose sleep over it. The bigger concern with higenamine is its own effect on heart rate and blood pressure, which matters a lot during chemo. Please run any supplement past your oncology team or pharmacist first.

What Is Netupitant and Palonosetron Used For?

Brand name: Akynzeo

Akynzeo capsules are used in adults to prevent both the short-term (acute) and longer-lasting (delayed) nausea and vomiting that can follow cancer chemotherapy — including, but not limited to, highly emetogenic (strongly nausea-causing) chemotherapy regimens. It is always taken together with the steroid dexamethasone.

Akynzeo capsules cover a broad range of chemotherapy regimens, including anthracycline plus cyclophosphamide-based regimens. Each ingredient targets a different stage: palonosetron addresses nausea and vomiting in the first 24 hours, while netupitant covers both the early and the delayed phase that can last several days after treatment.
