Hu Zhang and Imipramine

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Hu Zhang interacts with whole categories of medication. Imipramine falls into 3 of those categories, so it can interact with Hu Zhang in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Imipramine

Imipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP1A2.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP1A2 enzyme. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea with Hu Zhang and imipramine. Your body relies on a liver enzyme called CYP1A2 to help break down imipramine. The resveratrol in Hu Zhang may slow that enzyme down a bit, so in theory imipramine could clear more slowly and build up in your system.

If levels climbed, you might notice more of imipramine's usual side effects, things like drowsiness, dry mouth, constipation, blurry vision, or feeling lightheaded when you stand up. I want to be honest though: this concern comes from lab studies only and has not been reported in real people, so it's more theoretical than proven. Still, since imipramine needs careful dosing, I'd run any Hu Zhang use past your pharmacist or doctor first.

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

2 of 3

Imipramine also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP2C19.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2C19 enzyme. This interaction has not been reported in humans.

Dr. Brian Staiger, PharmD, BCPS

Here's the gist for you. Imipramine is an active medication that your body slowly clears, and one of the enzymes that helps break it down is called CYP2C19. The resveratrol in Hu Zhang can, at least in test-tube studies, slow that enzyme down. If that happens in real life, imipramine could clear more slowly and build up a bit higher than expected.

What might that feel like? More of the typical tricyclic side effects, like dry mouth, constipation, blurry vision, drowsiness, dizziness on standing, or a faster heartbeat. I want to be honest though: this is only a theoretical concern from lab data and hasn't been reported in actual patients. Please don't change anything on your own. Just give your pharmacist or doctor a heads-up so they can keep an eye on you.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Imipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with imipramine (Tofranil) and Hu Zhang. Your body partly clears imipramine using a liver enzyme called CYP3A4. Hu Zhang contains resveratrol, which in test-tube studies can slow that enzyme down. In theory, a slower enzyme means imipramine could build up a bit more than usual, which might mean stronger effects or more of its typical side effects: dry mouth, constipation, drowsiness, dizziness, blurred vision, or a faster heartbeat.

I want to keep this in perspective, though. This is a theoretical concern based mostly on lab work, and one human study using a large resveratrol dose didn't require any medication changes. So real-world risk looks low. Still, since imipramine affects the heart, it's worth a quick chat with your pharmacist or doctor before combining them.

“Imipramine (Tofranil) is an older tricyclic antidepressant, and the supplements on this page deserve real scrutiny before you add one. About Hu Zhang

What is Hu Zhang?

  • Source of resveratrol
  • Heart and circulation support
  • Inflammation
  • Liver and digestive complaints (traditional)
  • Cough and respiratory symptoms (traditional)
  • Skin and joint conditions (traditional topical use)

Hu Zhang (Japanese knotweed root) is a traditional Chinese herb that is one of the richest natural sources of resveratrol and emodin. Some lab and early human research looks interesting for inflammation and heart health, but high-quality evidence in people is limited. Talk to your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.

About Imipramine

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
