Humann Superbeets Heart Chews and Letrozole

Here is the interaction we found between these two, with the evidence behind it.

Minor

Cytochrome P450 3A4 (Cyp3A4) Substrates

Why this applies to Letrozole

Letrozole falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Humann Superbeets Heart Chews interacts with.

Each serving of HumanN SuperBeets includes 150mg of grape seed extract (2 chews). Laboratory studies suggest that grape seed extract might have inhibitory effects on the CYP3A4 metabolizing enzyme. If this enzyme is inhibited, drugs metabolized by it may undergo slower metabolism, potentially leading to (in many cases) higher-than-normal drug levels. Elevated drug levels can increase the risk of experiencing side effects. However, it's important to point out that there is no documented or reported instance of this specific interaction occurring in human studies.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with these chews and your letrozole (Femara). Letrozole is broken down by a liver enzyme called CYP3A4. The grape seed extract in SuperBeets Heart Chews (about 150mg per two chews) showed some ability to slow that same enzyme in lab dish studies. In theory, slowing it could let a bit more letrozole linger in your body, which might mean more of its usual effects like hot flashes, joint or muscle aches, fatigue, or headache.

That said, this is very weak, test-tube-only evidence, and there's no real-world case of it happening in people. So I wouldn't lose sleep over it. Still, since letrozole is an important cancer treatment, it's worth a quick mention to your oncologist or pharmacist before you start the chews.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Letrozole page

“Letrozole (Femara) is prescribed to keep hormone-driven breast cancer from returning, so a supplement that shifts its blood level can quietly undercut treatment. Give this list of 142 ingredients genuine respect, and learn the three major entries first.”

About Letrozole

What Is Letrozole Used For?

Brand name: Femara

Letrozole (brand name Femara, or available as generic letrozole tablets) is used to treat breast cancer in postmenopausal women whose tumors are hormone receptor positive — meaning the cancer feeds on estrogen. It is used in several situations: as an adjuvant treatment (therapy given after primary treatment to reduce the risk of the cancer returning) for early breast cancer, and as extended adjuvant treatment for women who have already completed five years of tamoxifen therapy.

Femara and letrozole tablets are also used as first-line or second-line treatment for locally advanced or metastatic (spread to other areas) breast cancer in postmenopausal women, including cases where the cancer has progressed after other anti-estrogen therapy.
