Mesoridazine Besylate falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.

Here's the concern with pairing these two. Mesoridazine (Serentil) is well known for stretching out the heart's electrical cycle, something we call QT prolongation. Iboga, through its compound ibogaine, has been linked in case reports to the very same effect. Put them together and you could be stacking two QT-prolonging agents on top of each other, which raises the risk of a dangerous, irregular heartbeat (including a rhythm called torsades de pointes that can cause fainting or worse).

To be fair, the evidence here is mostly anecdotal and partly theoretical, so it's not proven in a careful study. But mesoridazine is a strong QT drug on its own, and ibogaine carries real heart risks too, so this combination isn't one to take lightly. Please talk with your doctor or pharmacist before mixing them.

Theoretically, iboga might decrease the effectiveness of anticholinergic drugs.
In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.

Here's the idea on paper: mesoridazine (Serentil) has some anticholinergic properties, meaning part of how it works involves quieting the chemical messenger acetylcholine. Lab studies suggest Iboga might do the opposite, raising acetylcholine levels by blocking the enzyme that breaks it down. If that happened in the body, it could theoretically work against mesoridazine and make it less effective, so symptoms like agitation or restlessness might creep back.

I want to be honest about how thin this is. It's based only on test-tube research, and ibogaine, the main active compound in Iboga, did not show this effect. Far more important: Iboga can cause dangerous heart rhythm changes, and so can mesoridazine, so combining them is genuinely risky. Iboga is also illegal here. Please talk with your doctor or pharmacist before mixing anything with this medication.
