Serdexmethylphenidate Chloride, Dexmethylphenidate Hydrochloride falls under SEROTONERGIC DRUGS — the group Iboga interacts with.
Ibogaine, a constituent in iboga, has serotonergic effects. Theoretically, combining serotonergic drugs with iboga might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using iboga with serotonergic drugs.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Azstarys is a stimulant that nudges up brain chemicals like dopamine and norepinephrine, and it can gently raise serotonin too. Iboga (through its active compound ibogaine) also pushes serotonin levels higher. Put the two together and, in theory, you could stack up too much serotonin activity. That can lead to what we call serotonin syndrome, with symptoms like a racing heart, high blood pressure, sweating, shakiness, agitation, or confusion. Some of those overlap with the jitteriness and fast heartbeat stimulants can already cause.
I want to be honest: this warning is theoretical, based on how these two work, not on real case reports. That said, Iboga carries serious heart rhythm risks on its own and is illegal in the U.S. Please don't combine them without talking to your doctor or pharmacist first.

QT INTERVAL-PROLONGING DRUGS

Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with combining Iboga and Azstarys. Ibogaine, the main compound in Iboga, has been linked in several real-world case reports to a heart rhythm change called QT prolongation, which is a delay in how your heart's electrical system resets between beats. Azstarys is a stimulant, and stimulants can nudge your heart rate and rhythm too. Put them together and the worry is an additive effect on that QT interval, which in rare cases can trigger dangerous, irregular heartbeats.
To be fair, this interaction is mostly theoretical and based on scattered case reports, not strong studies. Still, Iboga carries serious heart risks on its own and is illegal in the U.S. Please don't change your ADHD medicine, but do talk with your doctor or pharmacist before mixing anything like this.
