These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Indian Long Pepper interacts with whole categories of medication. Estradiol, Progesterone falls into 2 of those categories, so it can interact with Indian Long Pepper in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Estradiol, Progesterone
Estradiol, Progesterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Indian Long Pepper interacts with.
In vitro research shows that piperine, a constituent of Indian long pepper, inhibits CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your body uses an enzyme called CYP3A4 to break down the estradiol and progesterone in Bijuva. The piperine in Indian long pepper may slow that enzyme down a little, at least based on early lab studies. In theory, that could let these hormones build up slightly in your blood, which might mean stronger effects or more side effects like breast tenderness, bloating, nausea, mood changes, or headaches.
I want to be honest with you though: this is only a theoretical concern from test-tube research, not from people, and the spice amounts you'd use in cooking are very unlikely to matter. Concentrated daily supplements are where I'd pay more attention. If you take pippali regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

P-GLYCOPROTEIN SUBSTRATES
2 of 2Estradiol, Progesterone also falls under P-GLYCOPROTEIN SUBSTRATES.
In vitro research shows that piperine, a constituent of Indian long pepper, can inhibit P-glycoprotein.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Bijuva (estradiol plus progesterone). Your body uses a transporter called P-glycoprotein to help move these hormones along. The piperine in Indian long pepper may block that transporter a bit, which could let slightly more hormone stay in your system. In theory, that might nudge up estrogen and progesterone effects, so you could notice things like breast tenderness, bloating, nausea, headaches, or mood changes.
I want to be honest with you though: this is only a theoretical concern based on lab studies, not real-world reports in people. A pinch of this spice in cooking almost certainly won't matter. Concentrated pippali supplements taken daily are where I'd pay attention. If you use it regularly, just give your pharmacist or doctor a quick heads-up so they can keep an eye on things.
