In vitro research shows that isopropylnorsynephrine is a highly selective, direct-acting beta-adrenergic receptor agonist with positive chronotropic effects, and hypertension has been reported in patients taking combination products containing isopropylnorsynephrine and other stimulants.

Here's the gist for Treprostinil (Orenitram). This medicine widens your blood vessels to ease the strain on the right side of your heart and lower the pressure in your lung arteries. Isopropylnorsynephrine is a stimulant that acts on beta-adrenergic receptors, and it can nudge your heart rate and blood pressure upward. So the worry is that it could work against what treprostinil is trying to do, possibly blunting your blood-pressure-lowering benefit or adding extra heart pounding.

I'll be honest, this concern is mostly theoretical and comes from lab data, not real human studies on this pair. Still, treprostinil is a serious medication for a serious condition, so don't add any pre-workout or weight-loss product with this ingredient without first checking with your doctor or pharmacist.

Isopropylnorsynephrine is a synthetic stimulant chemically related to synephrine, marketed mainly in weight-loss and pre-workout supplements. About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
