Phenylephrine falls under STIMULANT DRUGS — the group Isopropylnorsynephrine interacts with.
Isopropylnorsynephrine is an investigational beta-agonist and a derivative of synephrine and octopamine, which are stimulant constituents of bitter orange. In vitro research shows that isopropylnorsynephrine is a highly selective, direct-acting beta-adrenergic receptor agonist with positive chronotropic effects. Theoretically, isopropylnorsynephrine may also have stimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: both phenylephrine (the decongestant in Sudafed PE and similar cold and sinus products) and isopropylnorsynephrine are stimulants. Phenylephrine narrows blood vessels to ease a stuffy nose, while this supplement is thought to act on similar adrenergic receptors that nudge up heart rate and blood pressure. Stacking two stimulants together can add up, so the worry is mainly extra cardiovascular strain, things like a faster or pounding heartbeat, jitteriness, higher blood pressure, or headache.
I want to be honest about the evidence here: this is a theoretical concern based on how these compounds work, not proven cases. Still, since isopropylnorsynephrine has very little safety research behind it, I'd play it safe. Please check with your pharmacist or doctor before combining them, especially if you have any heart or blood pressure history.

What Is Phenylephrine Used For?

Phenylephrine is used in a number of different forms for different purposes. Intravenous (IV) phenylephrine — including products like Biorphen and Immphentiv — is given in hospital settings to raise blood pressure in adults when it drops dangerously low due to blood vessel widening (vasodilation), such as during anesthesia. Phenylephrine Hydrochloride Injection 10 mg/mL is also approved for use in septic shock (a life-threatening infection that causes a severe drop in blood pressure).
Nasal spray and drop products (such as Neo-Synephrine and Equate Nasal) temporarily relieve nasal congestion from colds, hay fever, or upper respiratory allergies. Ophthalmic (eye drop) solutions dilate the pupil for eye exams. Rectal suppository products (such as Preparation H and ReVIVOL-XR) help relieve itching, burning, and discomfort associated with hemorrhoids.
