Japanese Persimmon and Losartan Potassium

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Losartan Potassium

Losartan Potassium falls under ANTIHYPERTENSIVE DRUGS — the group Japanese Persimmon interacts with.

Japanese persimmon is thought to have hypotensive effects. Theoretically, concurrent use might increase risk of hypotension with drugs that lower blood pressure. These include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), amlodipine (Norvasc), hydrochlorothiazide (HydroDiuril), furosemide (Lasix), and many others.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: losartan (Cozaar) lowers your blood pressure, and Japanese persimmon may gently nudge it down too. Stack the two and those effects can add up, so your pressure could drop a bit more than you expect. That can show up as feeling lightheaded, dizzy, or woozy, especially when you stand up quickly, plus possible tiredness or a headache.

The good news is this is more of a theoretical, additive concern than a dangerous chemical clash, and losartan isn't a prodrug here, so persimmon won't make it weaker. Eating persimmon as a normal fruit is unlikely to cause trouble. Concentrated persimmon supplements or leaf extracts are where I'd be more cautious. If you use those regularly, check in with your pharmacist or doctor and keep an eye on your blood pressure.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan treats high blood pressure (hypertension) in adults and children 6 years and older. Keeping blood pressure under control lowers the risk of serious events like stroke and heart attack.

It is also used to reduce the risk of stroke in people who have high blood pressure and left ventricular hypertrophy (an enlarged heart muscle). Note: evidence suggests this stroke-risk benefit may not apply to Black patients. Additionally, losartan treats diabetic nephropathy — kidney damage caused by type 2 diabetes — in people who also have high blood pressure.
