These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Khella interacts with whole categories of medication. Sulfamethoxazole, Trimethoprim falls into 2 of those categories, so it can interact with Khella in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
PHOTOSENSITIZING DRUGS
1 of 2Why this applies to Sulfamethoxazole, Trimethoprim
Sulfamethoxazole, Trimethoprim falls under PHOTOSENSITIZING DRUGS — the group Khella interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: Bactrim (the sulfamethoxazole and trimethoprim combo) is a sulfa antibiotic, and sulfa drugs are already known to make your skin more sensitive to sunlight. Khella contains compounds (khellin and visnagin) that can do the same thing. So if you take them together, the worry is that you could burn more easily or react more strongly to the sun, think faster sunburn, redness, or a rash on areas that get sun exposure.
I want to be honest about the strength of this one. It's mostly theoretical and based on anecdotal reports, not strong studies, so it isn't a sure thing. Still, since both can add to sun sensitivity, it's smart to be cautious. Use sunscreen, cover up, and check with your pharmacist or doctor before combining them.

HEPATOTOXIC DRUGS
2 of 2Sulfamethoxazole, Trimethoprim also falls under HEPATOTOXIC DRUGS.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about pairing khella with Bactrim (sulfamethoxazole and trimethoprim). This isn't about one speeding up or slowing down the other. Instead, both can be tough on the liver, so taking them together may add up. Khella has been shown to raise liver enzymes (the transaminases your doctor checks on bloodwork), and Bactrim, on its own, can occasionally irritate the liver too.
I want to be honest that this is mostly a theoretical, just-in-case concern rather than something proven to happen often. Still, watch for warning signs like yellowing of your skin or eyes, dark urine, belly pain, or feeling unusually tired or nauseated. If any of that shows up, call your doctor. Before combining these, please run it by your pharmacist or physician.

About Khella
What is Khella?
- Asthma and bronchitis
- Angina and heart-related chest pain
- Kidney stones
- Vitiligo and psoriasis
- Mild high blood pressure
Khella is a Mediterranean herb whose seeds contain compounds like khellin that may relax smooth muscle in the airways and blood vessels. While it has a long history of traditional use for breathing and heart problems, modern evidence in supplement form is limited, and it can cause side effects and sun sensitivity, so talk with your pharmacist or doctor before using it.
