Kratom and Felbamate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CNS DEPRESSANTS

Why this applies to Felbamate

Felbamate falls under CNS DEPRESSANTS — the group Kratom interacts with.

Theoretically, concomitant use might increase the risk for adverse CNS depressant effects.
Kratom contains mitragynine, a mu-receptor agonist that may cause respiratory depression. Although results from animal research suggest that mitragynine causes less respiratory depression than the CNS depressant codeine, fatalities have been reported for patients who ingested a combination of kratom and O-desmethyltramadol, another mu-receptor agonist. Additionally, observational research suggests that adverse effects such as drowsiness and coma are more likely to occur in individuals taking kratom in combination with agents such as opioids and sedatives. Theoretically, ingesting kratom along with other CNS depressants can increase the risk of adverse effects, including drowsiness, coma, and/or severe or fatal respiratory depression.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind if you take Felbamate for your seizures and are thinking about kratom. Felbamate works by calming down overactive electrical activity in the brain, so it already has a mild settling or sedating effect for some people. Kratom acts on the brain's opioid receptors and can also slow things down, especially at higher doses.

Put the two together and that calming effect can stack up. In plain terms, you might feel extra drowsy, foggy, or slowed in your breathing. This concern is based on how these substances act in the body rather than solid studies in people, so it's theoretical. Still, real harm has happened when kratom was mixed with other depressants. Felbamate isn't a prodrug here, so this is about overlapping sedation. Please loop in your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Felbamate page

“Felbamate (Felbatol) is a seizure medicine usually reserved for epilepsy that other drugs have failed to control, in part because it can stress the liver on its own. That history is why its 172 supplement and herbal interactions deserve real vigilance, starting with the 14 rated major.”

About Felbamate

What Is Felbamate Used For?

Brand name: Felbatol

Felbamate is not a first-line seizure medication. It is used only when a person's epilepsy has not responded well enough to other treatments and the seizures are severe enough that the serious risks of this drug are considered acceptable.

In adults, felbamate can be used alone or alongside other antiepileptic drugs (AEDs) to treat partial seizures (seizures that start in one part of the brain), with or without spreading to become a full seizure. In children, felbamate is used as an add-on treatment for both partial and generalized seizures linked to Lennox-Gastaut syndrome, a severe form of childhood epilepsy. Felbatol is specifically labeled for these uses in both adults and children.
