Why this applies to Phenelzine Sulfate

Phenelzine Sulfate falls under CNS DEPRESSANTS — the group Kratom interacts with.

Theoretically, concomitant use might increase the risk for adverse CNS depressant effects.
Kratom contains mitragynine, a mu-receptor agonist that may cause respiratory depression. Although results from animal research suggest that mitragynine causes less respiratory depression than the CNS depressant codeine, fatalities have been reported for patients who ingested a combination of kratom and O-desmethyltramadol, another mu-receptor agonist. Additionally, observational research suggests that adverse effects such as drowsiness and coma are more likely to occur in individuals taking kratom in combination with agents such as opioids and sedatives. Theoretically, ingesting kratom along with other CNS depressants can increase the risk of adverse effects, including drowsiness, coma, and/or severe or fatal respiratory depression.

Here's the thing to keep in mind if you take phenelzine (Nardil) and are curious about kratom. On paper, both can press down on your central nervous system, so combining them may stack up effects like heavy drowsiness, slowed breathing, confusion, and in serious cases dangerously deep sedation. This concern comes from pharmacology and case reports, not from solid studies, so the evidence here is theoretical, but I wouldn't take it lightly.

There's an extra worry with phenelzine specifically. As an MAOI, it changes how your body handles many substances that act on the brain, and kratom's alkaloids work on opioid receptors and brain chemistry too. That mix raises the risk of unpredictable reactions. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first.

Theoretically, taking kratom in combination with serotonergic drugs may increase levels of serotonin, increasing the risk of serotonin syndrome.
A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the thing with Nardil (phenelzine): it's an MAOI, which means it already pushes serotonin and other brain chemicals to higher levels. Kratom acts on opioid receptors and also nudges serotonin upward. Stacking the two could pile on too much serotonin at once. The worry is serotonin syndrome, which can show up as agitation, a racing heart, sweating, shivering, muscle twitching, or confusion. MAOIs are also known for dangerous swings in blood pressure, so adding an unpredictable substance like kratom is genuinely risky.

To be fair, this is mostly theoretical and built on a single case report, not strong proof. But because phenelzine is such a touchy drug with serotonin, I'd treat this seriously. Please don't combine them without talking to your doctor or pharmacist first.

What Is Phenelzine Used For?

Brand name: Nardil

Phenelzine is used to treat depression — specifically the types described as atypical, nonendogenous, or neurotic depression. This often includes people who have a mix of anxiety and depression, or who have phobic or hypochondriacal features (strong fears about illness or physical symptoms).

It is generally not the first antidepressant a doctor will try. It is best suited for patients who have already tried other antidepressants without enough relief. There is less evidence supporting its use in severe depression with classic biological features (called endogenous depression).
