Theoretically, taking kratom in combination with serotonergic drugs may increase levels of serotonin, increasing the risk of serotonin syndrome.
A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the thing to know about pairing kratom with Tegaserod (Zelnorm): Tegaserod works by stimulating serotonin receptors in your gut to get things moving. Kratom also nudges serotonin activity in the body. When two products both push serotonin in the same direction, the worry is a rare but serious reaction called serotonin syndrome, with symptoms like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion.

I want to be honest about how strong this evidence is. It's mostly theoretical and based on a single case report involving several different medications, not Tegaserod specifically. So it's a real possibility, not a proven danger. Still, kratom carries its own risks and isn't regulated, so please check with your pharmacist or doctor before combining them.

What Is Tegaserod Used For?

  • Tegaserod is used in women younger than 65 years of age to treat irritable bowel syndrome with constipation (IBS-C; a condition that causes stomach pain or cramps, bloating, and infrequent or difficult passage of stools). Tegaserod is in a class of medications called serotonin agonists. It works by improving muscle movement and increasing production of fluid in the bowels.
