Treprostinil (Oral) falls under ANTIHYPERTENSIVE DRUGS — the group L-arginine interacts with.
L-arginine increases nitric oxide, which causes vasodilation. Clinical evidence shows that L-arginine can reduce blood pressure in some individuals with hypertension. Furthermore, combining L-arginine with some antihypertensive drugs seems to have additive vasodilating and blood pressure-lowering effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep an eye on with this pair. Oral treprostinil (Orenitram, often used for pulmonary arterial hypertension) works by relaxing and widening blood vessels to ease blood flow. L-arginine pushes in the same direction by boosting nitric oxide, which also relaxes vessels. Stack the two and their blood-pressure-lowering effects can add up.
What that might feel like is more dizziness, lightheadedness, flushing, headache, or feeling faint when you stand up. To be fair, this concern is theoretical, based on how both work rather than on hard studies with this exact combo. Still, since treprostinil is already a potent vasodilator, I'd run any L-arginine supplement past your doctor or pharmacist first before adding it.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for combining L-arginine with oral treprostinil (Remodulin). Treprostinil already works partly by relaxing blood vessels and making platelets less sticky, so it can raise your bleeding risk on its own. Early lab research hints that L-arginine may also make platelets a little less likely to clump together. Stack the two, and in theory you could see a small additional nudge toward easier bleeding or bruising.
I want to be honest about how soft this evidence is. It's theoretical, based on how these substances behave rather than real-world reports. Still, since treprostinil patients are watched closely, it's worth a quick chat with your pharmacist or doctor before adding an L-arginine supplement. Don't change anything on your own.

What Is Treprostinil Used For?
Brand names: Remodulin, Orenitram
Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.
Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
