L-tryptophan and Fentanyl citrate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

L-tryptophan interacts with whole categories of medication. Fentanyl citrate falls into 2 of those categories, so it can interact with L-tryptophan in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Major).

Major

CNS DEPRESSANTS

1 of 2

Why this applies to Fentanyl citrate

Fentanyl citrate falls under CNS DEPRESSANTS — the group L-tryptophan interacts with.

Theoretically, concomitant use of L-tryptophan with CNS depressants might cause additive sedative effects.
Clinical research shows that L-tryptophan can cause fatigue and drowsiness.
Severity Major Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: fentanyl is a strong opioid pain medicine that already slows down your brain and body. It can make you very sleepy, slow your breathing, and lower your alertness. L-tryptophan can also cause drowsiness and fatigue, so putting the two together may stack those calming effects on top of each other.

This isn't about fentanyl building up in your blood. It's more that both can push you toward feeling sedated at the same time, which could mean heavier drowsiness, foggy thinking, or dangerously slow breathing. With a medicine this powerful, even a small extra nudge matters. Please don't start, stop, or change anything on your own. Talk with your doctor or pharmacist first so they can guide you safely.

SEROTONERGIC DRUGS

2 of 2

Fentanyl citrate also falls under SEROTONERGIC DRUGS.

Theoretically, combining L-tryptophan with serotonergic drugs might cause additive serotonergic effects.
L-tryptophan is a precursor to serotonin. Theoretically, combining serotonergic drugs with L-tryptophan might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about pairing L-tryptophan with fentanyl: fentanyl is a strong opioid pain medicine, and besides its pain-relieving action it can also nudge serotonin levels up in the brain. L-tryptophan is a building block your body turns into serotonin. So when you stack the two, you may get a little extra serotonin activity than either one alone.

This worry is theoretical, based on how these substances work rather than lots of real-world reports, so I don't want to alarm you. Still, in rare cases too much serotonin can cause agitation, fast heartbeat, sweating, shivering, muscle twitching, or confusion. Don't start or stop anything on your own. Just run this combination past your doctor or pharmacist first so they can keep an eye on you.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Fentanyl citrate page

“The single most important thing to know about fentanyl citrate, an extremely potent opioid pain medicine, is that anything else with a sedating effect can deepen the slowed breathing that makes opioids dangerous. Alcohol, whether beer or wine, carries a major rating for exactly this reason, and 78 of the 223 ingredients on file add sed…”
