Why this applies to Clarithromycin
Clarithromycin falls under ANTIBIOTIC DRUGS — the group Lactobacillus Johnsonii interacts with.
Lactobacillus johnsonii preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. johnsonii preparations by at least two hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind with this pairing: clarithromycin is an antibiotic, and its whole job is to kill bacteria. A probiotic like Lactobacillus johnsonii is made up of live, friendly bacteria. So if you swallow them at the same time, the clarithromycin can wipe out a good chunk of those probiotic organisms before they ever get a chance to work. This doesn't make your antibiotic weaker or more dangerous, and it won't change how clarithromycin treats your infection. It just means the probiotic may not do much for you.
The easy fix is timing. Most pharmacists suggest spacing the two at least two hours apart. This is based on how the drugs work rather than hard studies, so it's more of a sensible precaution. If you're unsure, ask your pharmacist.

What Is Clarithromycin Used For?

Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.
Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
