Here is the interaction we found between these two, with the evidence behind it.
ANTIBIOTIC DRUGS
Why this applies to Rifampin
Rifampin falls under ANTIBIOTIC DRUGS — the group Lactobacillus Johnsonii interacts with.
Lactobacillus johnsonii preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. johnsonii preparations by at least two hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind with this combo: it's the probiotic that loses out, not your Rifampin. Rifampin is a powerful antibiotic, and it doesn't care whether bacteria are the harmful kind or the friendly Lactobacillus johnsonii in your supplement. Taken at the same time, it can kill off a good chunk of those live organisms before they ever get a chance to work.
So your TB or meningitis-prevention treatment with Rifampin still does its job just fine, you're simply getting less value out of the probiotic. This is a theory-based concern drawn from how these products behave, not from strong human studies. An easy fix is to space them apart by at least two hours. If you take both regularly, just run your timing past your pharmacist or doctor.

About Lactobacillus Johnsonii
What is Lactobacillus Johnsonii?
- Digestive and gut health
- Supporting healthy gut bacteria
- Diarrhea prevention or relief
- Immune support
- Helping with stomach (H. pylori) discomfort
Lactobacillus johnsonii is a probiotic bacterium found naturally in the human gut and in some fermented foods, and it is used mainly for digestive and immune support. Human evidence for most specific health claims is still limited, so its benefits are uncertain. It is generally well tolerated in healthy people, but anyone with a weakened immune system, serious illness, or who is pregnant should talk to a pharmacist or doctor first.
About Rifampin
What Is Rifampin Used For?
Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor
Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.
Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
