Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lemongrass interacts with.
Animal research shows that lemongrass and its constituent citral inhibit CYP3A4.

Here's the situation with Cabenuva, which combines cabotegravir and rilpivirine into a long-acting injection you usually get from your clinic. The rilpivirine part is partly cleared by a liver enzyme called CYP3A4. In lab and animal studies, lemongrass and its main aromatic compound, citral, seem to slow that enzyme down a bit. In theory, that could let rilpivirine linger longer in your body, which might mean more chance of side effects like headache, trouble sleeping, mood changes, or nausea.
I want to be honest, though — this is only based on test-tube and animal research, so it's a maybe, not a sure thing. A cup of lemongrass tea or using it as a cooking herb is unlikely to matter. Concentrated capsules or the essential oil are the bigger question. Since this is an HIV therapy, please check with your pharmacist or doctor before adding any lemongrass supplement.

GLUCURONIDATED DRUGS

Animal research shows that lemongrass and its constituent citral induce uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here is the gist for your Cabenuva (cabotegravir plus rilpivirine) injections. The cabotegravir part is cleared by your body mainly through a process called glucuronidation. In lab and animal studies, lemongrass (and its lemony compound citral) seems to speed up the enzyme that does this work. In theory, that could move cabotegravir out of your system a bit faster and nudge its levels down.
Why that matters here: with HIV treatment, keeping drug levels steady is important, and lower levels could make the medicine less effective over time. To be clear, this is theoretical and based only on animal data, not human studies, and small amounts of lemongrass in food or tea are unlikely to be an issue. Before using concentrated lemongrass supplements or the essential oil, please run it by your pharmacist or HIV provider.

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.
The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
