Bosentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lime interacts with.

Animal research shows that phenolics in lime juice inhibit cytochrome P450 3A4. This has been demonstrated in rats treated with sildenafil, a CYP3A4 substrate, and high doses of lime juice.

Here's the gist for Tracleer (bosentan): your body clears it largely through a liver enzyme called CYP3A4. Some lab and animal work suggests the plant compounds in lime juice can slow that same enzyme down. In theory, if that enzyme works slower, more bosentan could linger in your system, which might bump up its usual side effects like headache, flushing, swelling in the legs or ankles, or feeling lightheaded.

I want to keep this in perspective. This signal comes from rat studies using high amounts of lime juice, not from people, so it's far from proven. A squeeze of lime in your food or a drink almost certainly won't be an issue. Before using concentrated lime products, just run it past your pharmacist or doctor since bosentan already needs careful monitoring.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
