Here is the interaction we found between these two, with the evidence behind it.
LEVODOPA/CARBIDOPA (Sinemet)
One small clinical study suggests that octacosanol might worsen dyskinesias and increase nervous tension in some patients being treated with levodopa/carbidopa.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Stalevo combines levodopa (the part your brain turns into dopamine) with carbidopa and entacapone, two helpers that protect levodopa so more of it reaches your brain. If you take Parkinson's medicine like this, octacosanol is worth flagging. A small study hinted that it might worsen dyskinesias (those extra, involuntary movements like fidgeting or writhing) and leave some people feeling more wound up or tense.
I want to be honest: this is on the weaker, more theoretical end of the evidence, so it is not a sure thing. Still, since dyskinesias can already be tricky to balance with Stalevo, I would not add octacosanol on your own. Please check with your pharmacist or doctor first so they can watch your movements and mood.

“Most of what appears on this list works against Stalevo (carbidopa, levodopa, entacapone), the combination pill for Parkinson's disease, and that makes it a list worth genuine care. Everyday items like magnesium, iron, whey protein, and other amino acids can blunt levodopa's absorption or crowd it out of the brain, which shows up as wo…”
