Why this applies to Nintedanib Esylate

Nintedanib Esylate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Octacosanol interacts with.

Theoretically, octacosanol might inhibit platelet aggregation; however, clinical research suggests that this effect may not be clinically significant.
Octacosanol is the main component of policosanol. Some clinical research shows that taking policosanol 10-50 mg daily for 7-15 days can inhibit platelet aggregation in healthy patients. However, one clinical trial shows that taking policosanol 10 mg twice daily for 2 weeks prior to initiating warfarin does not affect warfarin pharmacokinetics or warfarin response. Furthermore, a study in patients undergoing percutaneous coronary intervention with a drug-eluting stent found that taking policosanol 40 mg plus clopidogrel and aspirin daily for 30 days modestly reduced the risk for minor bleeding events when compared with taking clopidogrel and aspirin alone.

Here's the thing to know about pairing octacosanol with Ofev (nintedanib). Octacosanol may have a mild blood-thinning effect by making platelets a little less sticky, though the research suggests this effect is weak and often not meaningful in real life. Nintedanib itself can already raise your risk of bleeding and bruising, so the worry is that stacking the two could add up.

In practice, what you might notice would be easier bruising, nosebleeds, bleeding gums, or cuts that take longer to stop. This is more of a theoretical, be-aware situation than a strong, proven danger, but since nintedanib already carries a bleeding caution, I'd run it past your doctor or pharmacist before adding octacosanol.

Octacosanol is a waxy long-chain alcohol from plant sources that is most often promoted for exercise endurance and cholesterol support, but solid human evidence is limited and results are mixed. It appears generally well tolerated in supplement amounts, but it is not a proven treatment for any condition, so talk with your pharmacist or doctor before using it.

What Is Nintedanib Used For?

Brand name: Ofev

Nintedanib capsules are approved for adults with idiopathic pulmonary fibrosis (IPF) — a chronic, progressive scarring of the lungs — and for adults with chronic fibrosing interstitial lung diseases (ILDs) with a progressive phenotype, meaning fibrosing lung diseases that continue to worsen over time despite treatment.

Nintedanib capsules (including Ofev) are also indicated to slow the rate of decline in lung function in adults with systemic sclerosis-associated interstitial lung disease (SSc-ILD), a lung complication of the autoimmune condition systemic sclerosis (scleroderma). Note that the Nintedanib esylate capsule product is approved for IPF and chronic fibrosing ILDs with a progressive phenotype, but SSc-ILD approval is carried by Ofev and other nintedanib capsule products — always confirm with your prescriber which product you have been prescribed.
