Here's the gist for this pairing: Troglitazone is a diabetes medicine that lowers your blood sugar by helping your body respond better to insulin. Octacosanol, which is the main piece of a related supplement called policosanol, has been reported in some studies to nudge blood sugar down a bit on its own. So the concern isn't that one blocks or speeds up the other in your liver. It's simply that two blood sugar lowering effects could stack up.

If they did add together, you might drift too low and feel shaky, sweaty, dizzy, or unusually tired. Honestly, the evidence here is thin and it's not even clear octacosanol does this the way policosanol does, so I wouldn't panic. Still, check in with your pharmacist or doctor before combining them, and keep an eye on your readings.
