Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Oregon Grape interacts with.

Theoretically, Oregon grape might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of Oregon grape, moderately inhibits cytochrome P450 3A4 (CYP3A4).

Here's the idea behind this pairing. Oregon grape contains berberine, which lab studies suggest can gently slow down a liver enzyme called CYP3A4. Rifampin happens to be handled in part by that enzyme, so in theory, blocking it could let a bit more rifampin linger in your system, which might mean slightly more of its usual effects like stomach upset, nausea, or strain on the liver.

That said, this is a theoretical concern based mostly on test-tube data, not real cases. Rifampin is actually a powerful enzyme booster itself, so the real-world impact is uncertain and likely small. Still, since rifampin is a serious drug, please run any Oregon grape supplement past your pharmacist or doctor first.

Oregon grape is a shrub whose root contains berberine and related compounds. The strongest (though still modest) evidence is for topical creams that may slightly ease psoriasis; evidence for other uses is limited. About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
