ADO-Trastuzumab Emtansine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist with Kadcyla (ado-trastuzumab emtansine). Once this antibody-drug conjugate releases its toxic payload (a compound called DM1) inside cancer cells, that payload is broken down partly by a liver enzyme known as CYP3A4. Phellodendron contains berberine, which lab and early human studies suggest can mildly slow CYP3A4 down.

The concern is theoretical: if that enzyme is blocked, the active payload could clear more slowly and build up, which might mean stronger side effects like low platelet counts, liver irritation, nerve tingling, or more fatigue and nausea. This is based on pharmacology, not real-world cases, so don't panic. Since Kadcyla is serious chemotherapy, please run any supplement, including phellodendron, past your oncology team first.

What Is Ado-trastuzumab Emtansine Injection Used For?

Brand name: Kadcyla

Kadcyla (ado-trastuzumab emtansine) is used to treat two types of HER2-positive breast cancer. In metastatic breast cancer — cancer that has spread to other parts of the body — it is used when patients have already received trastuzumab and a taxane chemotherapy, either together or separately.

Kadcyla is also used as an adjuvant (post-surgery) treatment for HER2-positive early breast cancer in patients who still had remaining invasive cancer after receiving neoadjuvant (pre-surgery) taxane- and trastuzumab-based treatment. Patients must be selected based on an FDA-authorized HER2 test before starting Kadcyla.
