Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

This combination pill mixes a few ingredients, and the one I'd watch here is butalbital, the barbiturate that does the heavy sedating work. Some of these components, butalbital and caffeine in particular, are partly handled by a liver enzyme called CYP3A4. Phellodendron (through its berberine content) may gently slow that enzyme down, so in theory your body could clear these drugs a little more slowly and let them build up.

If that happened, you might feel extra drowsy, foggy, dizzy, or unusually wired from the caffeine. Keep in mind this is only a theoretical concern based on lab data, not strong human studies, so don't panic. Still, since butalbital can be sedating, it's worth a quick chat with your pharmacist or doctor before pairing the two.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

The thing to focus on here is the aspirin in this combination product. Aspirin already thins your blood by keeping platelets from clumping together, which is exactly the same job that the berberine in phellodendron seems to do in lab and animal studies. Stack the two and you could, in theory, get a little extra bleeding tendency, like easier bruising, nosebleeds that take longer to stop, or stomach upset and bleeding.

I want to be honest that this is a theoretical concern based on test-tube and animal research, not solid human studies, so don't panic. Still, if you take this medicine regularly, check with your pharmacist or doctor before adding a phellodendron or berberine supplement, especially if you're on other blood thinners.
