The combo medicine you're asking about pairs aspirin with carisoprodol (a muscle relaxer) and codeine (an opioid). Both the carisoprodol and the codeine already work by calming the nervous system, so they can leave you drowsy, foggy, or slowed down. Phellodendron contains berberine, and in animal studies berberine seems to have its own mild sedative effect. Stacked together, the worry is simply more sleepiness than you bargained for.
I'll be honest with you: this is a theoretical concern based on how these things behave in the lab, not proven cases in people. Still, with codeine and a muscle relaxer on board, I'd play it safe. Watch for extra drowsiness, dizziness, or trouble concentrating, and check with your pharmacist or doctor before combining them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Here's a wrinkle worth knowing about. This combination pill contains codeine, and codeine is a prodrug. Your body has to convert it into morphine using a liver enzyme called CYP2D6 before it gives you much pain relief. Phellodendron (through its berberine content) may gently block that same enzyme.
So in theory, less codeine gets turned into active morphine, which could mean weaker pain control rather than stronger effects. The carisoprodol and aspirin parts aren't really affected by this. Keep in mind this is a theoretical concern based on lab work, not strong human studies, so it may not amount to much. Still, if your pain isn't well managed, mention the supplement to your pharmacist or doctor.

ANTICOAGULANT/ANTIPLATELET DRUGS

Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill (Soma Compound with Codeine) already contains aspirin, which thins the blood by keeping platelets from clumping together. Phellodendron supplies berberine, and in test-tube and animal studies berberine also seems to slow platelet activity. Stack the two together and you could nudge your bleeding risk a bit higher. That might show up as easier bruising, nosebleeds, bleeding gums, or stomach upset and stomach bleeding, which aspirin can cause on its own.
I want to be honest about the strength here: this is a theoretical concern based only on lab and animal work, not real human cases. The codeine and carisoprodol parts aren't really involved in this particular issue. Still, before you add this supplement, run it past your pharmacist or doctor, especially if you bleed easily or take it regularly.
