Budesonide, Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2D6.
Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Let me put your mind at ease a bit here. Your Symbicort inhaler combines two medicines, and the formoterol part can be processed by an enzyme called CYP2D6. Phellodendron contains berberine, which laboratory work suggests may slow that enzyme down. In theory, that could let a little more formoterol linger in your system, which might make side effects like a fast or pounding heartbeat, jitteriness, tremor, or trouble sleeping more noticeable.

I want to stress that this is theoretical, based on lab data rather than real cases. Inhaled doses are also quite small. Still, since the heart-racing effects are the ones to watch, it's worth a quick chat with your pharmacist or doctor before adding phellodendron. Please don't change your inhaler on your own.

Here's the gist with Symbicort, which pairs budesonide (an inhaled steroid) with formoterol (a long-acting bronchodilator). Phellodendron contains berberine, and early lab and human research hints that berberine may slow down a liver enzyme called CYP2C9. The thinking is that if that enzyme runs slower, certain medicines it processes could build up a bit more than usual.

I'll be honest though, this is a theoretical concern, not something proven in real patients on Symbicort. If levels did creep up, you might notice more of the usual side effects, like a fast or pounding heartbeat, shakiness, or jitteriness from the formoterol. The bigger point: since phellodendron is often sold in herb blends, just run it by your pharmacist or doctor before combining the two.
