Fomoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2D6.
Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Here's the idea with Phellodendron and your formoterol (Foradil). This bark contains berberine, which in lab studies and some early human research seems to slow down a liver enzyme called CYP2D6. Formoterol is partly cleared by that same pathway, so in theory, blocking it could let more of the drug linger in your system. Formoterol is an active medicine (not a prodrug that needs activating), so higher levels would mean a stronger effect.

What might that look like? The usual beta-agonist side effects nudged up a bit: a racing or pounding heart, jitteriness, shaky hands, or muscle cramps. To be clear, this is theoretical and based on pharmacology, not real-world reports. Still, please check with your pharmacist or doctor before pairing them.

Here's the gist for your Foradil. The compound that matters in phellodendron is berberine, which in early research can slow down a liver enzyme called CYP2C9. The idea is that if that enzyme is blocked, a drug it clears could build up a bit more than usual. With formoterol, that could in theory mean stronger or longer effects, and the side effects this inhaler is already known for, like a fast or pounding heartbeat, shakiness or tremor, headache, or feeling jittery.

I want to be honest though: this is theoretical, based on how things work in the lab, not on real cases of problems with formoterol. Formoterol is an active drug, not a prodrug, so the direction points toward slightly more effect, not less. Still, please run it by your pharmacist or doctor before adding phellodendron, especially if you notice any heart racing.

What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
