Nelfinavir Mesylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the simple version. Your body relies on a liver enzyme called CYP3A4 to clear nelfinavir (Viracept). Phellodendron contains berberine, which lab and early human studies suggest can partly block that same enzyme. If that blocking happens, your nelfinavir could clear more slowly and build up a bit, which may mean more of its usual side effects like diarrhea, nausea, gas, or stomach upset.

I want to be straight with you though: this concern is theoretical, based on how these compounds behave, not on real-world cases with nelfinavir. Still, HIV medicines work best at steady levels, so I wouldn't experiment on your own. Please check with your pharmacist or doctor before adding phellodendron, and don't change your nelfinavir dose.
