Niacin, Lovastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Advicor combines niacin with lovastatin, and it's the lovastatin part that matters here. Your body relies on an enzyme called CYP3A4 to clear lovastatin out of your system. The berberine in Phellodendron may gently slow that same enzyme down. So in theory, taking them together could let more lovastatin build up than usual.

If that happened, you'd be more likely to feel statin-type side effects, mainly muscle aches, soreness, or weakness, and rarely more serious muscle problems. I want to be honest though: this concern is theoretical, based on lab studies rather than real patient reports, so it may never cause trouble. Still, since muscle issues with statins are worth watching, please run this combo by your pharmacist or doctor before adding Phellodendron.
