Why this applies to Ombitasvir, Paritaprevir, Ritonavir, Dasabuvir
Ombitasvir, Paritaprevir, Ritonavir, Dasabuvir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Viekira Pak is a combination hepatitis C treatment, and several of its pieces, including the ritonavir booster, lean on the liver enzyme CYP3A4 for processing. The berberine in phellodendron may gently slow that same enzyme, so in theory it could nudge the levels of these drugs upward. Worth knowing: this regimen already contains ritonavir, which is itself a strong CYP3A4 blocker, so the practical impact of adding phellodendron is hard to predict.
If levels did rise, you might notice more nausea, tiredness, itching, or trouble sleeping. Just to be clear, this is a theoretical concern based on lab data, not proven harm in people. Still, please loop in your doctor or pharmacist before adding phellodendron, since keeping this hepatitis C course steady really matters.
