Why this applies to Tofacitinib
Tofacitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version with this pair. Your body relies on a liver enzyme called CYP3A4 to help clear tofacitinib (Xeljanz) from your system. The berberine in phellodendron may gently slow that enzyme down. If it does, tofacitinib could clear a little more slowly and build up, which may nudge up its effects and side effects, such as headaches, higher cholesterol, or a greater chance of infection, since this drug already dampens the immune system.
I want to be honest about how solid this is: it's theoretical, based on lab and pharmacology data rather than real-world reports, so it may not amount to much. Still, since tofacitinib is a serious medication, please run this combination by your pharmacist or doctor before mixing the two.

About Tofacitinib
What Is Tofacitinib Used For?
Brand names: Xeljanz, Xeljanz XR
Tofacitinib is used in adults with moderately to severely active rheumatoid arthritis, active psoriatic arthritis, active ankylosing spondylitis, and moderately to severely active ulcerative colitis. In each case it is for people who did not respond well to, or could not tolerate, one or more TNF blockers.
Xeljanz and tofacitinib tablets and oral solution are also used in children 2 years and older with active psoriatic arthritis or active polyarticular course juvenile idiopathic arthritis (arthritis in multiple joints). Extended-release tablets are labeled for adults only.
