Here's the gist: policosanol may nudge your blood sugar down a little on its own, and alogliptin (Nesina) is already working to lower it. Stack the two and the effects could add up, so your sugar might drop lower than expected. The good news is that DPP-4 inhibitors like alogliptin rarely cause low blood sugar by themselves, so the overall risk here is modest.

Still, keep an eye out for the usual signs of hypoglycemia: shakiness, sweating, dizziness, or feeling foggy. I'll be honest though, the evidence that policosanol really lowers cholesterol or sugar is shaky and not well confirmed outside a few studies. Before adding it, check in with your pharmacist or doctor so they can help you watch your numbers.

What is Policosanol?

Policosanol

Policosanol is a mix of waxy alcohols (mostly octacosanol) made from sugar cane or other plant waxes, and it is often marketed as a natural way to lower cholesterol. Early studies from Cuba showed strong cholesterol-lowering effects, but independent studies in other countries have mostly failed to confirm these benefits. Talk with your pharmacist or doctor before using it, especially if you take cholesterol medicine.

Alogliptin tablets — sold under the brand names Alogliptin and Nesina — are used along with diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus.

Alogliptin is not used for type 1 diabetes or diabetic ketoacidosis, as it would not be effective in those situations.
