Here's the gist: policosanol may nudge your blood sugar down on its own, and since metformin is already working to lower it, the two together could push your sugar lower than you intend. This isn't about one blocking or speeding up the other in the liver. They simply may pull in the same direction, so the concern is additive blood-sugar lowering.

Now, metformin by itself rarely causes true low blood sugar, so the risk here is modest. Still, keep an eye out for shakiness, sweating, dizziness, or feeling foggy, especially if you also take insulin or a sulfonylurea. Honestly, the evidence that policosanol even lowers cholesterol or glucose much is shaky outside of a few studies. Before adding it, just check with your pharmacist or doctor and keep monitoring your numbers.

Policosanol is a mix of waxy alcohols (mostly octacosanol) made from sugar cane or other plant waxes, and it is often marketed as a natural way to lower cholesterol. Early studies from Cuba showed strong cholesterol-lowering effects, but independent studies in other countries have mostly failed to confirm these benefits. Metformin is used to improve blood sugar (glucose) control in people with type 2 diabetes mellitus. It is always used together with diet and exercise, not instead of them.

Metformin hydrochloride tablets and oral solution are approved for adults and children 10 years and older. Metformin hydrochloride extended-release tablets — including brands such as Fortamet, Glumetza, and Metformin Hydrochloride Extended Release — are approved for adults only.
