Ibuprofen, Hydrocodone falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Policosanol interacts with.

Theoretically, taking policosanol with other antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
Some clinical research shows that taking policosanol 10-50 mg daily for 7-15 days can inhibit platelet aggregation in healthy patients. Although, one clinical trial shows that taking policosanol 10 mg twice daily for 2 weeks prior to warfarin dosing does not affect warfarin pharmacokinetics or warfarin response. Furthermore, a study in patients undergoing percutaneous coronary intervention with a drug-eluting stent found that taking policosanol 40 mg plus clopidogrel and aspirin daily for 30 days modestly reduced the risk for minor bleeding events when compared with taking clopidogrel and aspirin alone. Until more is known, use with caution.

Here's the thing to know about combining policosanol with this combo pill. The ibuprofen half is what matters most here, because ibuprofen already thins the blood a little and can be hard on the stomach lining. Policosanol may make your platelets (the cells that help blood clot) a bit less sticky, so stacking the two could, in theory, add up to easier bruising, nosebleeds, or stomach irritation and bleeding.

I want to be honest about the evidence, though. It's pretty mixed and on the weaker side, and one study even found slightly fewer minor bleeds. So this isn't a reason to panic. Still, if you notice unusual bruising, black or tarry stools, or bleeding that won't stop, loop in your pharmacist or doctor before changing anything.

Policosanol is a mix of waxy alcohols (mostly octacosanol) made from sugar cane or other plant waxes, and it is often marketed as a natural way to lower cholesterol. Early studies from Cuba showed strong cholesterol-lowering effects, but independent studies in other countries have mostly failed to confirm these benefits. Talk with your pharmacist or doctor before using it, especially if you take cholesterol medicine.
