Poria Mushroom and Cevimeline

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CHOLINERGIC DRUGS

Why this applies to Cevimeline

Cevimeline falls under CHOLINERGIC DRUGS — the group Poria Mushroom interacts with.

Theoretically, poria mushroom might have additive effects when used with cholinergic drugs.
In animal research, poria mushroom essential oil reduces acetylcholinesterase activity. This interaction has not been shown in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for you. Cevimeline (Evoxac) works by ramping up the cholinergic system to get your salivary glands flowing. Poria mushroom, in early animal studies, seems to slow down an enzyme called acetylcholinesterase, which normally breaks down acetylcholine. If that holds true in people, it could nudge cholinergic activity in the same direction as cevimeline, so the two might add together.

What might that feel like? More of cevimeline's usual effects, such as extra sweating, watery eyes, an upset stomach, more frequent bathroom trips, or feeling a bit flushed. To be honest, this is purely theoretical and hasn't been shown in humans, so I wouldn't panic. Just give your pharmacist or doctor a heads-up before combining them.

From our pharmacist’s overview on the Cevimeline page

“Cevimeline (Evoxac) stimulates saliva flow for the dry mouth of Sjögren's syndrome, and because the liver clears it through the CYP3A4 and CYP2D6 enzyme pathways, this interaction list warrants real attention. Three of the 169 ingredients rate major, and they come first: grapefruit can raise cevimeline levels, St.”

About Poria Mushroom

What is Poria Mushroom?

  • Fluid retention and swelling
  • Digestive complaints
  • Sleep and calming support
  • General wellness and immune support
  • Urinary support

Poria mushroom (Fu Ling) is a fungus long used in Traditional Chinese Medicine, mainly as a mild diuretic and digestive and calming aid. Modern scientific evidence in humans is very limited, so its benefits are not well proven. It is generally considered well tolerated, but talk with your pharmacist or doctor before using it, and avoid it in pregnancy and breastfeeding due to a lack of safety data.
