Propolis and Amoxicillin, Omeprazole Magnesium, Rifabutin

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Propolis interacts with whole categories of medication. Amoxicillin, Omeprazole Magnesium, Rifabutin falls into 3 of those categories, so it can interact with Propolis in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 3

Why this applies to Amoxicillin, Omeprazole Magnesium, Rifabutin

Amoxicillin, Omeprazole Magnesium, Rifabutin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2C19.
In vitro research shows that propolis extract can inhibit CYP2C19. However, animal research shows that propolis extract does not significantly affect CYP2C19 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C19 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Talicia is a three-in-one combo used to knock out H. pylori, and the part that matters here is the omeprazole. Omeprazole is handled by a liver enzyme called CYP2C19. In a test tube, propolis can slow that same enzyme down, which in theory could let omeprazole build up a bit. Higher omeprazole levels usually just mean more of its normal effects, things like headache, stomach upset, or loose stools.

Here's the reassuring part: this is only lab-dish data. In animal studies propolis didn't really change CYP2C19 in the body, likely because not much of it gets absorbed, and it has never been reported as a problem in people. For most folks, normal propolis use is unlikely to cause trouble. Still, it's worth a quick mention to your pharmacist or doctor.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 3

Amoxicillin, Omeprazole Magnesium, Rifabutin also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2C9.

Talicia is a three-in-one combo (amoxicillin, omeprazole, and rifabutin) used to knock out H. pylori stomach infections. The concern on file is that, in a test tube, propolis can block the liver enzyme CYP2C9, which could theoretically let certain medicines build up a bit. The good news here is that none of the three drugs in Talicia really lean on CYP2C9 to be cleared, so this particular pathway isn't a major route for them.

On top of that, the evidence is weak. Lab dishes showed some enzyme blocking, but animal studies didn't, and nothing has been seen in people. Everyday or food-level amounts of propolis are very unlikely to cause trouble. Still, if you take concentrated propolis supplements, it never hurts to mention it to your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Amoxicillin, Omeprazole Magnesium, Rifabutin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Talicia is a combo of three medicines used to treat H. pylori stomach infections. The one that matters here is rifabutin, which your liver clears using an enzyme called CYP3A4. In a test tube, some propolis compounds can block that enzyme, and in theory that could let rifabutin build up a bit, raising the chance of side effects like nausea, an upset stomach, or harmless orange-colored urine and tears.

Here's the honest part: this concern only comes from lab and animal studies, and it has never actually been seen in people. Real-world propolis doses likely don't reach the levels needed to matter. Still, if you take concentrated propolis supplements, mention it to your pharmacist or doctor so they can keep an eye on things.

  • Sore throat and mouth sores
  • Wound and skin healing
  • Immune support
  • Oral and gum health

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. It is generally well tolerated, but allergic reactions can occur, especially in people allergic to bee products.
