Why this applies to Deflazacort

Deflazacort falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the short version: your body relies on an enzyme called CYP3A4 to actually switch on deflazacort. After you swallow it, deflazacort is a prodrug, meaning it has to be converted into its active form before it can do its job of calming inflammation in Duchenne muscular dystrophy. In some lab dish studies, propolis appeared to slow that same enzyme down, which in theory could mean less of the active drug forms and a slightly weaker effect.

I want to be honest about how shaky this is. The evidence comes only from test tube and animal work, the results conflict, and it has never been seen in real people. Normal amounts of propolis are very unlikely to matter. Still, before adding concentrated propolis supplements, it's worth a quick word with your pharmacist or doctor.

Deflazacort treats Duchenne muscular dystrophy (DMD), a rare genetic muscle disease that causes progressive muscle weakness. It is available under several brand names, each with slightly different age approvals.

Emflaza and Jaythari (both tablets and oral suspension) are approved for patients 2 years of age and older. Deflazacort tablets, Deflazacort oral suspension, Kymbee (tablet and oral suspension), and Pyquvi (oral suspension) are approved for patients 5 years of age and older.
