Ashwagandha and Deflazacort

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Ashwagandha interacts with whole categories of medication. Deflazacort falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

IMMUNOSUPPRESSANTS

1 of 2

Why this applies to Deflazacort

Deflazacort falls under IMMUNOSUPPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might decrease the effects of immunosuppressants.
Ashwagandha has demonstrated immunostimulant effects in humans. Animal research has shown that ashwagandha can attenuate the immunosuppression caused by cyclophosphamide.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with this pairing. Deflazacort is a corticosteroid, and one of its main jobs is to calm down the immune system, which is part of how it helps in Duchenne muscular dystrophy. Ashwagandha tends to do the opposite. It has shown immune-stimulating effects, so in theory it could work against what deflazacort is trying to do and make the steroid a little less effective.

I want to be honest about how soft this evidence is. It comes mostly from lab and animal studies, not from people taking these two together, so it's more of a theoretical caution than a proven problem. Still, before you add ashwagandha, run it by your pharmacist or doctor so they can weigh in on your specific situation. Please don't stop or change your deflazacort on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Deflazacort also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version for Deflazacort (Emflaza). Your body activates and then clears deflazacort using a liver enzyme called CYP3A4. Some lab studies suggest ashwagandha may speed that enzyme up. In theory, a faster enzyme could clear the drug a bit quicker, which might slightly weaken its anti-inflammatory effect on the muscles in Duchenne muscular dystrophy.

I want to be honest about how soft this is. This concern comes only from test-tube research, not from studies in people, and it's rated a minor interaction. So I wouldn't panic. Still, since deflazacort is doing important work, it's worth a quick chat with your pharmacist or doctor before adding ashwagandha, rather than starting or stopping anything on your own.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Deflazacort page

“Deflazacort (Emflaza) is a corticosteroid, best known for treating Duchenne muscular dystrophy, and the liver clears it through the CYP3A4 enzyme, so supplements that block or speed up that enzyme can genuinely shift how much of the drug reaches the blood. That earns this page real caution.”
