CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Acetaminophen, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.
In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
In this combination cold medicine, the ingredient that matters here is dextromethorphan (the cough suppressant). Your body relies on a liver enzyme called CYP2D6 to clear it. Lab studies suggest quercetin can slow that enzyme down, which in theory could let dextromethorphan build up a bit. If that happened, you might notice more of its usual effects, like drowsiness, dizziness, or feeling lightheaded. The acetaminophen and pseudoephedrine parts aren't really affected by this.
Here's the reassuring part: this is only seen in test-tube research, not in real people, so I wouldn't lose sleep over it. Food amounts of quercetin are very unlikely to matter. If you're taking high-dose quercetin supplements, just mention it to your pharmacist or doctor so they can take a quick look.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Acetaminophen, Dextromethorphan, Pseudoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

This combination cold and flu product mixes three active ingredients, and the one that overlaps with quercetin's pathway is dextromethorphan (the cough suppressant), which the body partly handles through the CYP3A4 enzyme. The idea here is that quercetin may nudge this enzyme one way or the other, which could in theory change how much dextromethorphan stays in your system. If levels crept up, you might notice more drowsiness, dizziness, or a foggy feeling.
Honestly though, the evidence is thin. It comes mostly from lab and animal studies, and one human study even pointed the opposite direction. For an occasional dose of an over-the-counter cold remedy, I would not lose sleep over this. If you take quercetin supplements regularly, just mention it to your pharmacist so they can double-check for you.
