CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Chlorpheniramine, Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.
In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo product: the hydrocodone piece is interesting because it's actually a prodrug. Your body uses an enzyme called CYP2D6 to turn part of it into hydromorphone, which adds to the pain relief. Quercetin has been shown in lab dishes to slow that same enzyme down. In theory, blocking CYP2D6 could mean slightly less of that converted active form, so you might notice a bit less pain control from the hydrocodone.
Honestly though, this is only seen in test-tube studies and has never been reported in real people, so I wouldn't lose sleep over it, especially with food amounts of quercetin. If you take high-dose supplements alongside Tussend, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the angle worth knowing: the hydrocodone in Tussend is partly handled by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is that the research points both ways. In some studies quercetin blocks this enzyme (which could let hydrocodone build up a bit, meaning more drowsiness, dizziness, constipation, or slowed breathing), while another study suggests it may actually speed the enzyme up. Honestly, the evidence here is thin, mostly lab and animal work plus a couple of small human studies.
For most people, quercetin from foods like apples and onions isn't a concern. With concentrated supplements, it's a different story. Since hydrocodone is an opioid, I'd just run it by your pharmacist or doctor before pairing the two.
