Quercetin and Cyproterone Acetate, Ethinyl Estradiol

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Quercetin interacts with whole categories of medication. Cyproterone Acetate, Ethinyl Estradiol falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

P-GLYCOPROTEIN SUBSTRATES

1 of 2

Why this applies to Cyproterone Acetate, Ethinyl Estradiol

Cyproterone Acetate, Ethinyl Estradiol falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Diane-35 (cyproterone acetate plus ethinyl estradiol). Quercetin may block a gut transporter called P-glycoprotein, which can help certain medicines get absorbed a bit more. In theory, that could nudge up how much of your pill reaches your bloodstream. For a combined hormone product, slightly higher hormone levels might mean more of the usual side effects, things like nausea, breast tenderness, bloating, or headaches.

I want to be honest though: this is mostly theoretical. The evidence comes from small studies on other drugs, not this one, and results have been mixed. Everyday food amounts of quercetin are very unlikely to matter. If you're thinking about high-dose supplements, just check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Cyproterone Acetate, Ethinyl Estradiol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Both the cyproterone and the ethinyl estradiol in Diane-35 are processed by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme in either direction. In lab and small human studies, quercetin has sometimes slowed this enzyme (which could let hormone levels build up a bit, possibly meaning more nausea, breast tenderness, headaches, or mood changes) and sometimes sped it up (which could lower hormone levels). That mixed picture is the key point here.

To be honest, this is mostly theoretical and based on weak evidence, and occasional quercetin from foods like onions and apples is very unlikely to matter. With concentrated, high-dose supplements I'd be a little more cautious, since this pill is also your birth control. Please check with your pharmacist or doctor before starting a quercetin supplement.

  • Anti-inflammatory support
  • Immune support
  • Exercise recovery

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before taking high-dose supplements.
