CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Dextromethorphan, Guaifenesin, Pseudophedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.
In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Out of the three ingredients in this cough and cold combo, the one that matters here is the dextromethorphan. Your body relies on an enzyme called CYP2D6 to break it down. In lab studies, quercetin can slow that enzyme down a bit. If that happened in real life, dextromethorphan could build up and lead to more of its usual effects, like dizziness, drowsiness, or feeling a little foggy. Dextromethorphan is not a prodrug, so blocking the enzyme means more of it sticks around, not less.
The good news: this is only a theoretical concern based on test-tube research, and it has not been shown in people. Food amounts of quercetin are very unlikely to cause trouble. If you take high-dose quercetin supplements, just check with your pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Dextromethorphan, Guaifenesin, Pseudophedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

This combo cough and cold product has three parts, and the one that touches the liver enzyme CYP3A4 is the dextromethorphan (the cough suppressant). Quercetin can nudge that enzyme, but the lab and small human studies point in opposite directions, so the picture is fuzzy. In theory, if quercetin slows CYP3A4, a little more dextromethorphan could hang around, which might mean more drowsiness, dizziness, or stomach upset. If it speeds the enzyme up instead, the cough relief could fade a bit.
Honestly, this is mostly theoretical and built on weak evidence, and food amounts of quercetin are very unlikely to matter. If you take a high-dose supplement regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.
