Theoretically, concomitant use might increase the levels and adverse effects of CYP2D6 substrates.

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

In this combo cough and cold product, the piece that matters most here is the dextromethorphan (the cough suppressant). Your body uses a liver enzyme called CYP2D6 to clear it out. In test-tube studies, quercetin seems to slow that enzyme down. If that happened in people, dextromethorphan could build up a bit and you might feel more of its effects, like drowsiness, dizziness, or feeling foggy.

Here's the reassuring part: this has only been seen in lab studies, not in actual humans, so it's more of a theoretical worry than a proven problem. Everyday amounts of quercetin from foods are very unlikely to matter. If you take a high-dose quercetin supplement, just run it by your pharmacist or doctor first.

Dextromethorphan, Methscopolamine, Phenylephrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the short version: your cough and cold combo (dextromethorphan for cough, methscopolamine for drying, phenylephrine as a decongestant) is partly cleared by a liver enzyme called CYP3A4. Quercetin can nudge that enzyme, and honestly the lab data points in both directions. In some studies it slows the enzyme (which could let dextromethorphan build up a bit, meaning more drowsiness or dizziness), while one human study showed it actually sped the enzyme up.

The good news is this is based mostly on lab and animal work, so it's theoretical. Everyday food amounts of quercetin (apples, onions, tea) are very unlikely to matter. With concentrated supplements I'd just give your pharmacist a quick call before combining them.
