In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with quercetin and enasidenib (Idhifa). In lab dish studies, quercetin can block a liver enzyme called CYP2D6, which helps process certain drugs. If enasidenib is handled partly by that same enzyme, slowing it down could let a bit more of the drug build up in your body. In theory, that might mean stronger effects or more side effects, things like nausea, diarrhea, loss of appetite, or changes in your blood counts.
I want to be honest, though: this is only a theoretical concern seen in test tubes, not in real people, so I wouldn't lose sleep over it. Food amounts of quercetin are very unlikely to matter. Still, since enasidenib is a serious cancer medicine, please run any high-dose supplement by your oncologist or pharmacist first.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist: quercetin in supplement doses can slow down a liver enzyme called CYP2C9, and your leukemia medicine enasidenib (Idhifa) is partly handled by that same enzyme. If quercetin blocks it, your body may clear enasidenib a little more slowly, which could let levels creep up and possibly make side effects more likely. With enasidenib that might mean more nausea, diarrhea, tiredness, or changes in liver and bilirubin labs. Importantly, this is a serious cancer drug, so I'd want any change watched closely.
Keep in mind the evidence here is fairly theoretical, drawn from other CYP2C9 drugs and one small study, not from enasidenib itself. Everyday food amounts of quercetin are unlikely to matter. Before adding a high-dose supplement, please check with your oncologist or pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the situation with quercetin and your leukemia medicine, enasidenib (Idhifa). Enasidenib is partly handled by a liver enzyme called CYP3A4, and quercetin can nudge that enzyme. The tricky part is that quercetin pulls in two directions depending on the study. In some research it slows CYP3A4 down (which could let more enasidenib build up, potentially adding to side effects like nausea, low appetite, or a rise in bilirubin), while one human study suggests it can actually speed the enzyme up (which could lower drug levels a bit).
Honestly, this is mostly lab and theoretical data, so the real-world impact is uncertain. Small food amounts of quercetin are very unlikely to matter. Before starting a concentrated supplement during cancer treatment, please run it by your oncology team or pharmacist first.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the short version: your body clears enasidenib (Idhifa) partly through a liver enzyme called CYP2C8. Quercetin has shown, in test-tube studies only, that it can slow that same enzyme down. If it did that in real life, enasidenib could build up a bit more than expected, which might mean a higher chance of its known side effects, like nausea, low appetite, tiredness, or changes in your liver and bilirubin labs. Importantly, enasidenib is active on its own (not a prodrug), so more drug means stronger effects, not weaker.
The good news is this is theoretical. A human study with another CYP2C8 drug showed no real effect, so concentrated quercetin supplements matter more than food amounts. Since you're on cancer therapy, please clear any supplement with your oncology team or pharmacist first.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
