Why this applies to Ethinyl Estradiol, Norgestrel

Ethinyl Estradiol, Norgestrel falls under P-GLYCOPROTEIN SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the deal with your birth control pill (the ethinyl estradiol and norgestrel combo) and quercetin. Quercetin may block a transporter in your gut called P-glycoprotein, which helps move certain drugs in and out of your system. In theory, slowing that pump down could let a bit more of your pill's hormones get absorbed, which might mean slightly stronger effects like nausea, breast tenderness, bloating, or mood changes.

The honest truth is this is mostly theoretical for your pill. The evidence comes from small studies on other drugs, not on birth control, and food amounts of quercetin almost certainly won't matter. If you want to take a high-dose supplement, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Ethinyl Estradiol, Norgestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Your birth control pill (Ovral, Lo/Ovral, and similar combos) contains ethinyl estradiol and norgestrel, both of which your body processes partly through a liver enzyme called CYP3A4. Quercetin can nudge that enzyme, but the lab and small human studies point in different directions. Some research suggests quercetin slows CYP3A4 (which could raise hormone levels and bump up side effects like nausea, breast tenderness, or spotting), while other data hint it may speed the enzyme up (which could theoretically lower hormone levels).

Here is the honest truth: this is mostly test-tube and animal evidence, so it is theoretical and not well proven in people. The amounts of quercetin you get from food are very unlikely to matter. If you are considering high-dose quercetin supplements, just run it by your pharmacist or doctor first.
