CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Fomoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.
In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with formoterol (Foradil) and quercetin. Your liver uses an enzyme called CYP2D6 to help break down formoterol. In test-tube studies, quercetin can slow that enzyme down a bit. If that happened in your body, formoterol could hang around longer and its effects might get stronger.
With a long-acting beta agonist like this, stronger effects could mean things like a racing or pounding heartbeat, shaky hands, jitteriness, or a headache. That said, this is purely theoretical, coming from lab work, not from real people, so I wouldn't lose sleep over it. Everyday food amounts of quercetin are very unlikely to matter. If you're thinking about high-dose supplements, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the honest part: this interaction on file is built around quercetin slowing down a liver enzyme called CYP2C9, which could let drugs cleared by that pathway build up. The catch is that formoterol isn't really handled by CYP2C9 in a big way. Your inhaled formoterol is mostly broken down by a different process (glucuronidation) and clears out fairly directly, so this particular enzyme story doesn't fit it cleanly.
Because of that, I'd call the practical risk here pretty low and largely theoretical for formoterol specifically. Food amounts of quercetin almost certainly won't matter. If you're considering high-dose quercetin supplements, just run it by your pharmacist or doctor so they can double-check against your full medication list and your COPD plan.

What Is Formoterol Oral Inhalation Used For?
Brand name: Foradil
Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.
Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
