In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
If you're taking this combo pill (the ibuprofen plus hydrocodone product), here's the simple version. Quercetin can block a kidney transporter called OAT3, which helps clear certain drugs out of your body. The ibuprofen part leans on transporters like this, so in theory quercetin could slow its exit and let a little more build up. That might mean more of ibuprofen's usual side effects, like stomach upset, heartburn, or, with regular use, more strain on the kidneys.
Now the honest part: this is based only on lab studies, and it has never actually been reported in people. Everyday food amounts of quercetin are very unlikely to matter. If you're thinking about high-dose supplements, just run it by your pharmacist or doctor first.

Here's the short version on combining quercetin with this ibuprofen and hydrocodone combo product. Quercetin can block a kidney transporter called OAT1, which helps clear certain drugs out through your urine. The ibuprofen part of this pill leans on transporters like that to leave your body. So in theory, if quercetin slows that exit, ibuprofen could hang around a bit longer and its effects could be slightly stronger.
What that might look like in real life is more of the usual ibuprofen side effects, such as stomach upset, heartburn, or, with heavier or longer use, kidney strain or higher blood pressure. I want to be honest, though: this is only seen in lab studies, never reported in actual people. So it's worth mentioning to your pharmacist or doctor, but it isn't a reason to panic.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

This combo product pairs ibuprofen (a pain reliever) with hydrocodone, an opioid. The piece that matters here is hydrocodone, because your body relies on an enzyme called CYP2D6 to convert it into hydromorphone, the form that gives much of its pain relief. In lab dishes, quercetin can block CYP2D6. If that same blocking happened in people, it could actually lessen hydrocodone's pain-relieving punch, since less of it gets activated.
The honest part: this is only test-tube data and has never been shown in humans, so the real-world effect is likely small, especially with food amounts of quercetin. Still, if you take high-dose quercetin supplements and notice your pain isn't well controlled, check in with your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

This combo pill pairs ibuprofen (an anti-inflammatory) with hydrocodone (an opioid pain reliever). The hydrocodone part is partly handled by a liver enzyme called CYP3A4, and quercetin has shown mixed effects on that enzyme in lab and small human studies. In theory, if quercetin slows CYP3A4, more hydrocodone could hang around, which might mean extra drowsiness, dizziness, constipation, or slowed breathing. If it instead speeds the enzyme up, your pain relief could feel weaker.
Honestly, the evidence here is thin and points in both directions, so I wouldn't lose sleep over an occasional quercetin-rich meal. Concentrated supplement doses are the bigger question mark. If you take daily quercetin capsules, just run it by your pharmacist or doctor first.
