Quercetin and Iptacopan

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES

Why this applies to Iptacopan

Iptacopan falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C8 substrates.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the situation with Fabhalta (iptacopan) and quercetin. Iptacopan is partly cleared by a liver enzyme called CYP2C8. In test-tube studies, quercetin can slow that same enzyme down. If that happened in your body, iptacopan could build up a bit, which might mean more of its usual side effects like headache, belly pain, diarrhea, or nausea.

The important part is that this is theoretical, based only on lab work, not real people taking these two together. In fact, a human study with another CYP2C8 drug showed no meaningful effect. Everyday food amounts of quercetin are very unlikely to matter. Concentrated supplements are the bigger question mark, so before adding one, check with your pharmacist or doctor. Don't change your iptacopan on your own.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Iptacopan page

“Someone taking iptacopan (Fabhalta) for paroxysmal nocturnal hemoglobinuria relies on a twice-daily oral capsule holding steady blood levels, and that is exactly what most of these 28 supplements can disturb, so this list earns genuine care. The lone major entry, activated charcoal, can adsorb the drug in the gut and cut its exposure,…”

About Quercetin

What is Quercetin?

Quercetin
  • Seasonal allergies
  • Antioxidant support
  • Heart and blood pressure health
  • Anti-inflammatory support
  • Immune support
  • Exercise recovery

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before taking high-dose supplements.
